What is an 8D report?

An 8D report (8 Disciplines) is a structured approach to problem-solving and quality assurance, where you identify recurring problems and find solutions that ensure they do not repeat.

The 8D report is often used in connection with complaint cases between customers and suppliers, where the customer experiences recurring discrepancies/problems with products, services or processes. With the 8D report, it ensures that the supplier not only reacts to the specific discrepancies but also manages to identify and eliminate the underlying causes (the root of the discrepancies), so the problems do not recur.

It’s therefore not just about resolving the issue here and now, but also about implementing permanent preventative measures with the supplier, so something similar cannot occur in the future. Therefore, the 8D report is often a requirement from the customer, for the cooperation with the supplier to continue.

Our 8D problem-solving course systematically follows the steps in the 8D method, where we go through the theory and teach you to use the various tools associated with each step. We do this using concrete examples from Danish and international companies, as well as through training exercises and case work (your own case), allowing you to work with the theory in practice.

During the course, all participants will have the opportunity to work with a concrete, practical problem from their own company, preferably in close dialogue with the person who has the strategic responsibility for the issue at hand (and thus has an in-depth knowledge of the problem). It could, for example, be: customer complaints you have received or warranty problems that indicate larger error frequencies than expected.

Note: Do not choose problems under time pressure – you should ideally have peace of mind to focus on the learning process.

  • Although the method is primarily used in relation to complaints against external suppliers, it can also be used internally in an organisation, ensuring a uniform project structure. This makes it easier to communicate and compare solutions across the organisation if everyone is speaking from the same foundation. This not only makes good sense, it will ultimately also save you a lot of time and resources.
  • The 8D report can also be a good tool to reduce your costs associated with poor quality (Cost of Poor Quality), and enable you to deliver the right products/services in the quality your customers demand.
  • As a supplier, it's also a good signal to send that you are willing to work proactively to solve your problems. An 8D can therefore, even though it is created because of problems, actually strengthen the customer relationship in the long run.
  • So whether you are the customer experiencing errors and discrepancies, or you are the supplier asked to fill in an 8D, this course will be valuable for you.

Content of the course

Duration: 3 teaching days (2 whole days and two half-days) divided into 4 modules. The teaching takes place as physical classroom teaching on small closed groups (max 16 participants).

Here you will learn how to follow the 8D process steps while working in a cross-functional team (Cross Functional Team – CFT). You will practice using the correct problem-solving tools to identify the root of the problems and eliminate it through permanent corrective actions.

During the course, there will be a strong focus on the methods for verifying the underlying causes and confirming the effect of the permanent corrective actions.

Module 1

  • Presentation of practical cases brought by participants
  • 8D process step by step
  • Typical timeline for the 8D process (customer requirements)
  • 8D compared with other problem-solving strategies and how they relate.
  • Step D1: Team formation - planning of own case
  • Step D2: Problem description (own case)
  • Step D3: Implementation of containment actions – on own case

Module 2

  • We go through steps D1-D3 in the 8D model based on participants' own cases
  • Step D4: Learn to identify the root of the problem and verify the result (own case)
  • Step D5: Development of permanent corrective actions (own case)
  • Step D6: Implementation of permanent corrective actions and validation of actions (own case)
  • Step D7: Prevent the problem from recurring (own case)
  • Step D8: Recognise and congratulate the team. The team should be formally thanked by the organisation

Module 3

  • Individual coaching related to practical cases (or with the team if necessary / desired)

Module 4

  • Each participant/team presents their case to the class
  • Feedback from the class
  • Teacher's evaluation and feedback on cases
  • Summary and questions

Outcome – What competences does it provide?

  • You learn to prepare an 8D report step by step
  • Understand the relationship between 8D and Failure Mode and Effects Analysis (FMEA)
  • Facilitate an effective 8D report
  • How you become a valuable participant in 8D teams
  • Learn to define the root of the problems (root cause) and identify triggers of faults
  • Set up a cross-functional team (CFT – Cross Functional Team)
  • Determine an effective containment strategy, which limits the scope of the problem until the main cause is found (interim containment actions)
  • Identify the underlying causes and evaluate the effect of the changes you make (Corrective actions). These changes should hopefully solve the problems permanently.
  • Manage and save 8D content (experiences) for future use
  • Understand when to use tools like: "5 x Why", "Fishbone Analysis", "Is/Is not" problem-solving analysis, Statistical Process Control (SPC), Poka Yoke (fail-safing).
  • Understand correlation with control plan method (Plan documenting all changes and has an action plan to be able to maintain the implemented solutions moving forward).
